DETERMINISTIC BLACKJACK - FAIL-CLOSED INTEGRITY RECOVERY

If RUN_END_TO_END_INTEGRITY_GATE.bat fails, normal Replay and CLI/live startup remain blocked.
The gate writes GUI\INTEGRITY_GATE_LAST_FAILURE.txt naming the failed layer and safe recovery rules.

Recovery is repair/restore + rerun, never bypass:
- accidental code/file drift: restore the affected file from the last accepted package;
- intentional code change: compile/test/audit it, then deliberately establish a new accepted release baseline;
- historical evidence mismatch: investigate provenance/authority; never edit observed evidence merely to satisfy the gate;
- plate-remediation mismatch: reconcile against the audited remediation register/authority matrix;
- package-hygiene failure: remove prohibited distribution content rather than weakening the check.

A stale failure report is deleted automatically after a complete PASS.
